Completing the DA 3355 form is an important step in the promotion process for soldiers in the United States Army Reserve. This form helps determine eligibility for promotion based on various criteria, including military training, awards, and education. Here’s how to fill it out correctly.
- Start with your personal information: Fill in your name, date of birth in the format YYYYMMDD, and the date of filling out the form.
- Enter your organization: Specify the unit or organization you belong to.
- Recommended grade and MOS: Indicate the grade you are being recommended for and your Military Occupational Specialty (MOS).
- Section A - Military Training:
- Fill in your latest Army Physical Fitness Test (APFT) date and raw scores for push-ups, sit-ups, and the run. Calculate and enter the total points earned.
- Provide details of your latest weapons qualification, including the date, DA Form used, total hits, and points earned.
- Sum up the total military training points earned and enter this in the designated space.
- Commander’s section: The commander must sign and date the form in the specified blocks.
- Soldier's signature: Sign and date the form in the recommended soldier's signature block.
- Promotion board recommendation: Select “Yes” or “No” regarding your recommendation for the promotion board.
- Board president's section: The board president must also sign and date the form.
- Section B - Administrative Points:
- List all awards, decorations, and badges along with their order and award numbers. Calculate the total points earned.
- Document your military education, including all relevant courses, and total the points earned.
- Include any civilian education, certifications, or language proficiency tests, and total those points as well.
- Section C - Totals: Fill in the points earned for each category and ensure the totals are correct.
- Section D - Authentication: An administrative official must certify the total points. Include their name, signature, grade, and date. The promotion authority must also sign and date the form.
- Counseling statement: If applicable, the soldier must sign and date the counseling statement regarding their promotion status.
Once the form is completed, it should be submitted through the appropriate channels for review. Make sure to keep a copy for your records and ensure all information is accurate to avoid any delays in the promotion process.
