Completing the Delta Property form is essential for initiating a claim regarding lost luggage. After ensuring that five days have passed since your luggage went missing, you can proceed with filling out the form. Be thorough and accurate in your responses to facilitate the processing of your claim.
- Fill in the PIR# and Mailing Date at the top of the form.
- Provide your Name (Mr, Mrs, Miss/Ms) and contact numbers, including Home Phone and Business Phone.
- Enter your Home Address and Business Address, including City, State, Zip/Postal Code, and Country.
- Input your Email Address for correspondence.
- Fill in the Claim Check Number(s) and Baggage File Reference number.
- Indicate if you are a SkyMiles Member and provide your SkyMiles # and SkyMiles Status.
- Detail your complete itinerary, including From, To, Airline, Flight Number, and Date.
- Specify the number of pieces checked and the number missing.
- Estimate the weight of each missing piece.
- Indicate where you checked your luggage and if it was checked under another name.
- Provide information on where you last saw your luggage.
- State if Delta was notified of the loss immediately and provide details if applicable.
- Indicate if the loss was reported to any other airlines.
- Explain any delay in reporting the loss, if applicable.
- Answer questions regarding customs and excess valuation.
- Describe your luggage, including Type, Brand, Color, Material, and any features.
- List all articles contained in the luggage, providing details such as Qty, Article, Color, Size, Material, Store & City, Date Purchased, and Original Cost.
- Provide any additional comments, including previous claims with Delta or other airlines.
- Sign and date the form, ensuring that all information is accurate and complete.
- Retain a copy of all documents for your records.
- Mail the original documents within 21 days from the date of loss to the specified address.
