- List the name and contact information of the event or activity leader, including their phone number and email address.
- In the Participant section, enter the participant's full name, date of birth, and age. Provide primary and secondary telephone numbers, including home, cell, or work options.
- Complete the participant's address, including city and state or province.
- Fill in the emergency contact information for a parent or guardian, including their primary and secondary telephone numbers.
- Indicate whether the participant requires a special diet. If yes, explain the dietary restrictions.
- Answer whether the participant has any allergies. If yes, list the allergies.
- State if the participant is taking any medication or over-the-counter drugs. Specify if they can self-administer their medication.
- If applicable, describe any chronic or recurring illnesses the participant has.
- Note if the participant has had surgery or a serious illness in the past year and provide details if necessary.
- Identify any limits, restrictions, or disabilities that may affect the participant's ability to participate fully in the event.
- List any other accommodations or special needs the participant may have that the event planner should be aware of.
