Once you have decided to suspend your Melaleuca Preferred Customer benefits, completing the cancellation form is the next step. This process involves providing your personal information, sharing feedback regarding your decision, and signing the form to confirm your request. Here is how to fill out the form step-by-step:
- Begin with Customer Information: Clearly print your Melaleuca customer number, telephone number, first name, middle initial, last name, and complete address including city, state, and ZIP code.
- Provide Feedback: Choose one or more reasons for your decision to suspend your benefits by marking the appropriate box. You can select from options such as personal circumstances, financial reasons, or overstocking issues. If you select "Other," write your feedback in the space provided.
- Sign the Form: Your signature and date are required, as well as your spouse's signature if applicable. Clearly state your intention to cancel the Preferred Customer Agreement and acknowledge the loss of benefits. If you want to suspend additional subscriptions, check the corresponding box.
- Submit the Form: Decide how you will send the form. You can mail it to Melaleuca at 3910 S. Yellowstone Hwy., Idaho Falls, ID 83402-6003, fax it to (888) 528-2090, or email it to myaccount@melaleuca.com. Keep in mind that requests received after the 25th of the month will be processed the following month.
After submitting your form, your request will be processed, and you will lose access to your Preferred Customer benefits. However, should you decide to reactivate your membership within six months, the membership fee will be waived. During this time, you can still shop at regular prices whenever you wish.
