Instructions on Utilizing Ptcas
Completing the PTCAS form is a crucial step in your journey toward a career in physical therapy. This form requires accurate and thorough information about your observation hours, which will be verified by a licensed physical therapist. After gathering the necessary details and having them signed, you will be ready to submit your application through the PTCAS system.
- Start by obtaining a blank PTCAS Physical Therapy Observation Hours Verification Form.
- Fill in your name in the "Name of Applicant" field.
- Leave the "PTCAS ID#" field blank for now.
- Provide the name of the facility where you gained your observation hours.
- Enter the street address, city, state, zip/postal code, and country of the facility.
- Write the name of the physical therapist who will verify your hours.
- Include the PT's license number and the state where the license was issued.
- Fill in the PT's phone number and email address.
- Indicate whether the PT will also submit a reference by selecting "Yes" or "No."
- Specify the type of experience you had: Paid, Volunteer, or Both.
- Enter the start date and end date of your observation hours.
- Check all applicable inpatient settings and enter the hours completed and hours planned/in-progress for each setting.
- Check all applicable outpatient settings and enter the hours completed and hours planned/in-progress for each setting.
- Calculate and enter the total number of hours completed for all settings.
- Check all patient diagnoses or populations you observed during your experience.
- Have the physical therapist complete the recommendation section, indicating their assessment of your potential as a healthcare provider.
- Ensure the physical therapist signs and dates the form.
- Once the form is completed and signed, print the new (bar-coded) PTCAS verification form.
- Attach the signed verification form to the new PTCAS verification form.
- Send both forms in a single envelope to PTCAS.
