Completing the Transfer Request form is a crucial step in ensuring that the service contract for your motorcycle is properly transferred to the new owner. After filling out the form, it should be mailed to the designated address along with the required payment. Below are the steps to help you fill out the form accurately.
- Start by entering the date on which the motorcycle changed possession.
- In the section labeled Transfer From, provide the original owner's information, including their name, street address, city, state, and zip code.
- Next, fill out the Transfer To section with the new owner's details, including their name, street address, city, state, and zip code.
- Include the area code and phone number for both the original owner and the new owner.
- Write down the Vehicle Identification Number (VIN) of the motorcycle.
- Enter the contract number associated with the service contract.
- Indicate the motorcycle mileage on the date of transfer.
- As the new owner, sign the form to acknowledge that you have received all necessary repair orders and receipts from the original owner.
- Also, sign to confirm that you have received written verification of the remaining Harley-Davidson® factory warranty, if applicable.
- Attach a check for the $25 transfer fee, made payable to the Harley-Davidson® Service Center, to the completed form.
- Finally, mail the completed Transfer Request form and payment to the Operations Department at the specified address: Operations Department, Harley-Davidson® Service Center, P. O. Box 2840, Scottsdale, AZ 85252-2840.
